ATTENTION ALL PLAYERS: The following forms are required by the school prior to tryouts:

The State of CT DOE Health Assessment Record must be on file by the first day of tryouts. If you have the current form on file you do not need to resubmit.  A current form is valid for one year only.

The WHS Emergency Treatment Card must be completed and returned to the school by the first day of tryouts.
